Inside look into what went wrong. Basically a system screw-up.
I was never a huge fan of the conspiracy theory, just that there was fail and they had to fix it. Clearly, they are. These things do happen and it was just unfortunate that Amazon's PR didn't really do a lot to answer questions earlier. From the sounds of the second link, they were working on it as soon as they'd heard. If they'd told people that, I don't think everyone would have gotten quite as angry.
